Procedure

To change the name of a form:

Start the Designer.
In the Schema Repository Explorer view, expand the Record Types folder and then expand the node of the record type whose form name you want to change.
Expand the Forms folder.
Right-click the form name and then click Rename.
Enter a new name for the form.
